That’s where Prowers Medical Center’s Home Health team comes in. Our top priority is to assist patients in their homes, enabling them to live a safe, healthy, and well-balanced lifestyle. Through the commitment to their patients, the Home Health team is making a difference.
Our experienced and dedicated nursing and rehabilitation teams use a multidisciplinary approach to ensure the highest quality of home health skilled services. We strive to ensure you or your loved one reaches their maximum health potential. Our goal is to keep you at home as long as possible by teaching you independent living skills.
Utilizing a team effort, Prowers Medical Center Home Health works cooperatively with patients, their physicians and their family. Under the direction of the patient’s doctor, we coordinate healthcare services, provide supplies and equipment as necessary, and help manage medication to promote recovery at home. In addition to top-quality health care services, we bring the human element to you. We understand the complexities of illnesses, and the far-reaching impacts on you and your family. We are always willing to go the extra mile. We respect your rights and personalize your plan of care in the comfort of your home. Our main goal is returning you to your independence.
