When a patient no longer needs acute medical care, but isn’t quite able to go home, a transitional program (Swing Bed) may be available.

Your attending provider will be key in deciding if transition care will be necessary. A Swing Bed patient is discharged as an inpatient and readmitted as a skilled nursing Swing Bed patient.

The patient stays in the same area of the hospital but is encouraged and expected to participate in activities of daily living. Prowers Medical Center also accepts Swing Bed referrals from other facilities to ensure that our community members can receive rehabilitative services in the comfort of their hometown.

Medicare* patients must meet the following criteria:

  • Medicare Part A

  • Qualifying stay in acute care for a minimum of three consecutive midnights

  • Must have rehabilitation potential determined by a provider

  • Clinical staff must be able to meet your needs

  • * Some commercial insurances are accepted
